Initial Assessment
If you or your loved one has already been enrolled in the waiver, our attendant care staff will coordinate a free in-home visit to assess your loved one's needs. If you are not yet enrolled in the waiver, you can contact us so our professional and dedicated staff will assist and guide you through the waiver application process and enrollment.

Our Adult Home Care program includes assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, and mobility. We also provide companionship, medication reminders, light housekeeping, meal preparation, and health monitoring — all delivered in the comfort of your loved one's home.
Adults aged 65 and older who are low-income and have a medical condition, disability, or age-related limitation that affects their ability to live independently may qualify. Eligibility is typically determined by Medicaid, and our team can help you through the entire application and approval process. Those under 65 years old and pediatric cases can also be accepted based on eligibility and care requirements.
