Our Services
Adult Day Health Care (ADHC) is a structured daytime program that provides health, social, and therapeutic services to adults who need supervised care but wish to remain living at home. It is ideal for seniors or adults with physical, cognitive, or medical needs, and it also gives family caregivers a much-needed respite during the day.
Our program includes nursing care and health monitoring, social and recreational activities, nutritious meals and snacks, medication management, transportation to and from our center, and bilingual case management to coordinate benefits and community resources.
Yes. We offer door-to-door transportation for participants at our Louisville location. Our vehicles are equipped and our drivers are trained to assist individuals with mobility needs. Transportation is part of the care plan and is coordinated directly with your family.
Yes. Our entire team is bilingual in English and Spanish. We understand that many families in Kentucky's Hispanic community feel more comfortable communicating in Spanish, and we are proud to offer full care and support in both languages — from intake to daily care to case management.
Admissions & Medicaid
Once you or your loved one receives the Waiver enrollment approval letter, contact our intake care coordinators or visit any of our four locations for assistance. Our team will help determine whether a free in-home or in-center assessment is needed to understand the participant's needs, health history, and goals. If you are seeking case management services, our professional team will assist you in developing a Person-Centered Service Plan (PCSP) and guide you through any required paperwork.
No referral is required to contact us or schedule an assessment. However, a physician's order may be needed depending on the level of care and benefits involved. Our team will let you know exactly what documentation is needed and help you obtain it if necessary.
Little to no wait time. Once your case manager has authorized services with Buena Vida, most participants can begin attending or receiving our other services the next day. If Medicaid or Waiver enrollment is needed, the timeline depends on state processing times. We will guide you every step of the way and keep you informed throughout the process.
Family Involvement
Yes. Family members are always welcome to visit our centers during operating hours. We encourage family involvement and believe it plays an important role in the well-being of each participant. Please call ahead so we can best prepare for your visit.
Our team maintains open communication with families through regular updates, care plan meetings, and direct contact with our nursing and case management staff. You can reach us by phone during business hours and we will promptly address any questions or concerns you have about your loved one's care.
Our program gives family caregivers reliable daytime respite so they can work, rest, or attend to other responsibilities knowing their loved one is safe and well-cared for. Many families tell us that Buena Vida not only improves the life of their loved one, but also reduces caregiver stress and burnout significantly.
Health & Safety
We serve adults with a wide range of conditions including diabetes, hypertension, heart disease, COPD, stroke recovery, Alzheimer's and other forms of dementia, Parkinson's disease, and mobility limitations. Our nursing team monitors each participant's health daily and coordinates with their primary care physicians as needed. Pediatric cases are also served through Home Care services.
Our centers are staffed with registered nurses who are trained in emergency response. In the event of a medical emergency, we immediately contact 911, notify the family, and accompany the participant to the hospital if needed. All staff are CPR and first aid certified, and we have clear emergency protocols in place at every location.
Yes. Buena Vida Adult Day Health Center is fully licensed by the Kentucky Cabinet for Health and Family Services and operates in compliance with all state and federal regulations. We undergo regular inspections and are committed to maintaining the highest standards of safety, cleanliness, and quality care at all four locations.
